Seleziona un metodo di pagamento means choose a way of payment. I wonder why it is in Italian.
Anayway the only way of payment is paypal. There is not a different from being from USA or elsewhere as it is a PDF so there are not shipping cost.
Let me know if you still have trouble, in case I can sedn you specific paypal bill
